QUESTIONS FOR PARENTS TO MONITOR STUDENTS

  • In the last 24 hours, has your child experienced a new loss of taste or smell?

  • In the last 24 hours, has your child experienced a fever of over 100.4 degrees?

  • In the last 24 hours, has your child experienced a new or worsening cough?

  • In the last 24 hours, has your child experienced shortness of breath?

  • In the last 24 hours, has your child experienced a sore throat?

  • In the last 24 hours, has your child experienced diarrhea?

  • Has your child had close contact with someone diagnosed with COVID-19? Close contact means a household member, or someone within 6 feet for longer than 10 minutes?

  • Has your child been directed or told by the local health department or their healthcare provider to self-isolate or self-quarantine?

  • If you answered yes to any of the questions you should monitor your child for continuation of symptoms of COVID and follow your local health department or family physician's recommendations for testing.
